Facility Use Request Form

This form is for any organization, group, or individual who would like to reserve space in the facility or equipment. Requests will be discussed for approval at the next staff meeting based on the information provided on this form. Please fill in all the requested information as thoroughly as possible.

This form is not for wedding, baptism, or confirmation requests.

Filling out this form is not a guarantee of approval. Staff will be in contact with you to confirm the details of your request. Requests for facility and equipment use such as recurring Bible studies, special events, etc. must be approved prior to announcing publicly. All requests will be followed up on by staff as to the approval and or/changes needed to your request as soon as possible.

Security and/or cleaning deposits may be required.
